Newborn Screening Life Changer Brochure Order Form

All birthing hospitals should have received their January 2025 shipment of Newborn Screening brochures.  Please contact NBS at [email protected] for the tracking number if your facility has not yet received NBS brochures.

Each quarter the Newborn Screening Program will send educational resources to each facility automatically using the quarterly average of births from the previous year. If additional resources are needed before the next shipment, please request more below. Most hospitals include these brochures in the hospital welcome packet. Click here to view the digital copy.

"Your Baby Needs Another Hearing Test" Packet Order Form

These packets should be given to each parent/caregiver of a baby who does not pass the hearing screen inpatient. The folder allows the parent to keep all hearing screen information in one place and contains the “Your Baby Needs Another Hearing Test” brochure, hearing roadmap, CDC Hearing Factsheet,  and “Can you tell which Baby has Hearing Loss” Risk Factor brochure. Hearing Screen Stickers Order Form

These stickers can be used to notify parents and/or hospital staff that a hearing screen has been completed and results should be shared.
